This form is intended for use by students who have been formally admitted to USU. Students who have not been admitted should contact the USU Admissions Office.
You may be asked for a transcript from your previous institution, a catalog description for the course, a syllabus, or other relevant class materials to help the academic department articulate the course appropriately.
